Please Note:

To ensure you take full advantage of your appointment with Dr. Cameron, please fill out the appropriate form and bring it with you to your appointment. Please be as specific and thorough as possible in all information provided, even if you do not think that it pertains to your reasoning for coming to see us.

If the Patient Application is incomplete at the time of your appointment, we will have to reschedule due to lack of time needed to complete your exam. Please arrive to your appointment 10-15 minutes early so that we can help you get checked in for your appointment and answer any questions you may have regarding your application. We ask that you also bring your I.D. and health insurance card, as well as any information that may pertain to your case.

Please DO NOT wear any metal or plastic such as jewelry, zippers, buttons, or clasps as it will interfere with x-rays, if they are deemed necessary after your exam is completed. We have athletic clothing for you to change into, or you may wear/bring your own. Please wear tight, form-fitting clothing for your appointment! (Compression shorts/pants, spandex, tight shirts, sports bra without clasps, etc.) This will help us to have the most accurate examination, and will eliminate time spent changing, taking jewelry off, etc.

You will receive a reminder call the day before your appointment, but if an emergency or appointment conflict comes up, please give us at least 24-hours’ notice to avoid losing your deposit.

We are located at 1717 W. Francis Ave, Ste. 202. Spokane, WA 99205, which is on the corner of Francis and Ash across the street from the 5-Mile shopping center. There is a bottom and upper entrance. Please proceed on the bottom entrance, which will lead you up to the upper parking area of the building. Please feel free to give us a call with any questions or concerns at (509) 443-3535.
